By signing below, as parent or guardian of the registered student(s), I do hereby give permission for (student/s) to travel to the Teen Leadership Conference Retreat at Washington Family Ranch in Antelope, OR from December 30, 2024 to January 1, 2025 in the company of Sunset Bible Church youth ministry staff. I understand the permission to include emergency medical treatment in case I/we cannot be reached in a medical emergency.
